Hätäensiapu - First Aid

Kävin torstaina taas ensiapukurssin kertauspäivän ja on se aina hyödyllistä. Pitäjä oli sama ja oheinen 10v sitten julkaisemani kalvo oli tälläkin kertaa esillä.

Tämä oli kuulemma Anderssille 1504. vedetty EA-kurssi ja muistelisin olleeni opetettavanaan jo 90-luvulla. Että asiasta olisi itsellenikin hyötyä, niin tässä teillekin tiedoksi muutama ohje.

Last Thursday I refreshed my First Aid training and noticed again how important it is. The teacher was the same guy who showed me the slide below 10 years ago.

This was his 1504th First Aid training and I'd recall that he taught me already in the 90s. For that the training would possibly benefit me too, here's some instructions for you too.

Sekalaisia vinkkejä/ohjeita:

• Puristussiteeksi isoihin verenvuotoihin kannattaa mukana olla CAT-T puristusside (löytyy Suomestakin, mutta kalliimmalla). Tuon saa kiinni vaikka itselleenkin yhdellä kädellä.
• Kännykkään kannattaa asentaa 112 Suomi -sovellus. Hätäpuhelun yhteydessä välittyy tarkka sijainti ja vaaratiedotteet viranomaisilta tulevat kohdealueella oleville.
• Leffoista poiketen kolarissa yksi auto tuhannesta syttyy palamaan. Anna uhrin olla autossa ellei vaaraa. Varmista tietysti hengitys.
• Irronnut peukalo tms. --> puhdas muovipussi --> umpeen ja kylmään veteen
• Murtumassa, akillesvammassa tms. tärkeää laittaa heti kylmähoitoa alueelle. Turvotus voi estää leikkauksen moneksi päiväksi.
• Kaikki hoito vaikeutuu 10x jos potilaalla hypotermia --> lämpöpeite toimii oikeasti, kannattaa olla aina mukana!
• Älä soita mihinkään hätäpuhelun jälkeen, että puhelin pysyy avoimena jos soittavat takaisin

Miscellaneous tips/hints:

• For big bleeding wounds the best help is CAT-T Tournique. You can even put it to your own arm with one hand.
• At least in Finland there's an '112 Suomi' app that provides your exact location while making the emergency call. It also delivers emergency notification according to your location.
• Unlike in movies, only one car of thousand burns in accidents. You can leave the victim there to wait for help. Just make sure she/he breaths.
• A severed finger etc --> clean plastic bag --> air tightly closed and to cold water
• For broken bone or achilles tendon etc it's very important to start the cold treatment. If it swells operating it can be delayed even for several days.
• All the treatments change to 10x more difficult if the patient is in hypothermia, so it's good to always carry an emergency blanket with you. It really works!
• Don't make other phone calls after calling to 112 (or 911) as they may need to call you back

Välitön ensiapu:

1. Varmista hengitys
1. Ravista, herääkö?
2. Kokeile hengitys kämmenen selällä (pulssia turha kokeilla)
3. Avaa hengitystiet  
4. Älä kaiva sormella syvempää
5. Hukkuneelle viisi nopeaa puhallusta ennen elvytystä
6. Tukehtumistilanteessa soita heti 112. Jos apu tepsii, niin hälytyksen voi perua.
7. Heimlich-otteessa samalla käske yskimään
2. Soita hätänumeroon 112, kuuntele ohjeet 
3. Joku hakemaan lähintä defibrillaattoria
1. Ainoa keino lopettaa kammiovärinä
2. Ei voi tehdä vahinkoa
4. Tajuton hengittävä kylkiasentoon (kts ylempi kuva, vasen kylki parempi!)
5. Elvytys
1. Puhallukset käytännössä turhaa, joten jätä tekemättä
2. Painalluksia rintalastaan 100/minuutti, kunnes apu tulee paikalle tai potilas herää
6. Iso verenvuoto
1. Painetaan nyrkillä, ei sormella
2. CAT-T puristusside aina raajan juureen
3. Sisäisessä verenvuodossa ei jalkoja ylös, vaan vaaka-asento

The First Aid:

1. Make sure the victim is breathing
1. Shake to wake up her/him
2. Try the breathing with the back of your hand (it's no use to search for pulse)
3. Free respiratory tract
4. Don't touch deep in the throat with your finger
5. For somebody drowned give five fast blows before starting reflation
6. On choking case call 112 (911) at first, it your actions help the call can be cancelled
7. In Heimlich manouver tell also to cough hard
2. Call 112 (/911) and do as told
3. Send someone to get a defibrillator
1. It's the only way to stop ventricular fibrillation
2. It can't harm the patient
4. If the victim is breathing put her/him on ones side (on the left side, see the pic above)
5. Reflation
1. Blowing air is futile in practise so don't do that
2. Push the chest 100 times per minute until the help arrives or the patient wakes up
6. Major bleeding
1. Press with the fist, no fingers
2. CAT-T tourniquet goes at the base of the limb
3. If there's internal bleeding don't raise the legs, but keep the patient in horizontal position
